Clinic Application

This Terms and Agreement applies to all clinic applications submitted through the CavClinics platform. By continuing with this application, you acknowledge and accept the terms below.

1. Accuracy of Information

   You confirm that all submitted data, declarations, and uploaded files are accurate, complete, and lawfully owned or authorized by your clinic.

2. Verification and Compliance Review

   CavClinics may review and verify your application details with applicable government or regulatory references when required for validation.

3. Approval Requirement

   Clinic listings will only be published after successful review and formal approval by CavClinics administrators.

4. Document and Profile Maintenance

   You are responsible for keeping clinic records up to date, including permits, contact details, operating schedules, and relevant profile information.

5. Additional Requirements

   CavClinics may request further supporting documents or clarifications, and may place an application on hold or reject it if requirements are incomplete or inconsistent.

6. Use of Submitted Data

   Submitted clinic and representative information may be used for verification, account setup, operational processing, audit logging, and approved public listing.

7. Platform Integrity

   Fraud, falsification, unauthorized access, or misuse of the CavClinics system may result in denial of application, account suspension, or permanent removal.
